What is the Adrenal gland?

The adrenal glands are part of the endocrine system and are located on top of your kidneys as can be seen in the images. They are responsible for numerous hormone related functions in the body and proper adrenal health is therefore very important.

The glands have a medulla in the center surrounded by the cortex. The adrenal cortex produces hormones that controls sex (androgens, estrogens), salt balance in the blood (aldosterone), and sugar balance (cortisol). The adrenal medulla produces hormones involved in the fight-or-flight response (catecholamines, or adrenaline type hormones such as epinephrine and norepinephrine). 

The three layers of the adrenal cortex are:

  • The zona glomerulosa (ZG) is the most superficial layer of the adrenal cortex and it produces the hormone aldosterone as well as some small amounts of progesterone (a sex hormone). The mineralocorticoid aldosterone is produced here.
  • The zona fasciculata (ZF) is the middle zone of the adrenal cortex, and it primarily produces cortisol.
  • The zona reticularis (ZR) is the inner most zone of the adrenal cortex and it is adjacent to the adrenal medulla. Functions of the zona retularis are to store cholesterol for steroidogenesis (the making of steroid hormones) and the secretion of sex hormones such as estrogen, and testosterone (in small amounts).

Symptoms & diagnosis of adrenal disorders

Some of the most common symptoms of adrenal disorders include:

  • High blood pressure: Hypertension is one of the most common symptoms of overactive adrenal glands. Tumors that produce catecholamines, aldosterone or cortisol all lead to this due to disruption in salt regulating functions of these hormones.
  • Conn’s syndrome: which involves excess aldosterone production can show up with muscle cramps and heart palpitations apart from hypertension.
  • Cushing’s Syndrome: Caused by excess cortisol, the symptoms show up as fat deposits above the clavicle, a moon face, central fat deposition, weight gain, stretch marks, purple striae on the abdomen and muscle weakness in the extremities. 
  • Excess production of medullar hormones shows up pallor, tremors, weight loss, panic and anxiety.
  • In cases of tumors etc. there may be additional symptoms like abdominal pain.

Approach to treating adrenal disorders

Adrenal gland disorders can develop when:

  • your pituitary gland fails to control your hormone production properly
  • benign, or noncancerous, tumors grow in your adrenal glands
  • malignant, or cancerous, tumors grow in your adrenal glands
  • infections develop in your adrenal glands
  • you inherit certain genetic mutations

To diagnose the issues, the first line is usually to order certain blood tests that will check for:

  • adrenal hormones
  • pituitary hormones
  • glucose
  • potassium
  • sodium

Imaging tests like an X-ray or MRI may then be recommended which will show certain changes that help confirm the diagnoses.

If you are diagnosed with an adrenal gland disorder, Dr.  Shehla will prescribe treatment to help your adrenal glands function properly again. The possible lines of treatment include hormone replacement therapy if you have low adrenal function, such as that caused by Addison’s disease. Medications as well as radiation treatment may be recommended if your glands are producing too much of a hormone.
